Kindergarten Speech Therapy “narrative language group”

Term 2, 2024
6 Week program running Weeks 3, 4, 5, 6, 7, 8

• I give permission for my child to participate in a 1 hour, 6 week, language and phonics group run by a speech and language pathologist, Rebecca McKeon, at school on a weekly basis during weeks 3, 4, 5, 6, 7 and 8 of term 2, 2024.
• I understand there is a total cost of $480 payable by myself for these group sessions and payment will need to made in full by the 11th May 2024 directly to Starfish Speech Pathology to secure my child’s spot.
• Starfish Speech Pathology will send through an invoice for the 6 sessions, once the permission form has been received.
• I understand that these sessions are non-refundable, but every effort will be made to make p sessions missed by my child.
• I understand I will be issued with a receipt for each session individually as it is conducted to allow me to make any health fund or Medicare claims for which I am eligible.
